How does it work?

It heals you either 7% of your maximum health or 20% of the damage you took in the previous 5 seconds if it is more than 7% of your max health.
Sustained damage is damage that you took (I don't believe that damage absorbed is factored in) |

It is damage you actually take, not damage you absorb. So if you want to prepare for the big spiky damage you'd actually should let smaller attacks blunder you prior to the big attack. The technique is known as "spiking" and is one of the secret tools DK's have been using on raids for a while.
