Reaching Renown 7 and Renown 22 unlocks a quest at the Renown NPC in your covenant hall which must be completed before the PVP gear upgrades become available. Once you have completed this quest, upgrades each cost a specific amount of honor.
Conquest gear also requires you to have reached a specific rank and currently have a specific rating. If you earn the achievement for reaching a rating but then you drop your rating, you won't be able to upgrade.

We were made aware of an issue that caused the ranking on certain PvP items looted from the Great Vault to be listed incorrectly, making them unable to be upgraded. This has since been resolved, but the fix is not retroactive. If you were affected by this issue, please contact us and we may be able to assist.
