A Titan Prison Fragment will only drop during the Timewalking difficulty of Ulduar. You must be on the quest Disturbance Detected: Ulduar to receive it.
Note: The Fragment must be looted manually - it is not automatically pushed to your inventory. The Postmaster does not mail unlooted quest items.
