Unable to link Xbox Account - "Provider did not accept user authorization"

Users may encounter the error message "Provider did not accept user authorization" when attempting to connect an Xbox account with a Battle.net account. This error occurs in the following scenarios:
- The Microsoft account used on your Xbox does not have an associated Xbox Live account.
- The Microsoft account used is a Microsoft child account or has Parental Controls enabled. Child accounts with Parental Controls are not supported for linking.
