Wrong Rewards Offered In The Great Vault

The number of reward slots available in the Great Vault, along with the Item Level of the rewards offered, is determined by the quantity and difficulty rating of the activities you complete to unlock them:

The Item Level of rewards for all three activities is based on the highest difficulty tier you accomplished in each activity per the number of completions required to unlock that reward slot.

Raids Example: If you kill 2 Heroic bosses during the week, and 4 Normal bosses, only one of your vault slots will have the higher Heroic Item Level. Your other two vault slots will have a lower Item Level appropriate for Normal difficulty bosses.

Dungeons Example: If you finish 1 Mythic dungeon during the week, and 3 Heroic dungeons, one vault slot will have a higher Item Level reward to reflect the Mythic dungeon, while the other will have a lower Item Level to match the Heroic difficulty dungeons

World Example: If you finished 2 Delves, killed a world boss, and finished a world event, you will have unlocked two vault slots. The first will have an Item Level appropriate for the lowest difficulty Delve you completed, and the other will have an Item Level appropriate for world activities.

In all of the above cases, repeatedly completing higher difficulty challenges equal to the number of completions required to unlock each Great Vault slot, will increase the Item Level of the rewards offered.

Mythic Keystone Information

If you finish a Mythic keystone dungeon in time during the week, the next week’s Great Vault will contain a keystone of that same level.

For example, if your highest Mythic keystone completion during week one is a +15 that you completed in time, you will get another +15 keystone from the Great Vault to start week two. If you run that +15 during week two and fail to meet the timer, however, and complete no other +15 (or higher) Mythic Keystone dungeons the rest of week two, you will receive a +14 keystone from the Great Vault to start week three.

IF your Keystone Seems lower level than it should be

If the Great Vault gave you keystone of the wrong level it is likely because you were awarded an old reward. The Great Vault saves any rewards you don't loot, and shows you the rewards in order from oldest to most recent. You can see if the rewards you're receiving are old from a message above the Collect button.

You can only have one keystone per week. This means that, if the Great Vault already gave you an older keystone, you cannot receive a second, newer keystone, even if the newer keystone is better.
